Gold Star Collaborative Mission

The Mission of the Gold Star Collaborative is to determine that applicable providers have met agreed upon Gold Star Shared Expectations. These expectations are above minimum compliance and correlate to quality services delivered to individuals with developmental disabilities in Stark County. Collaborative members also deliver suggestions, networking and peer mentoring to other community providers to promote and enhance the quality of the service delivery system.

Collaborative’s Objectives:

Increase family/guardian and individual satisfaction with services (National Core Indicators data)
Improvement of provider Direct Support Professional retention (provider annual turnover rate)
Identify system-wide changes & issues impacting individuals served and how providers can most effectively manage them in a positive manner.
Improve the quality of the service delivery system in Stark County through improved provider relations, increases in provider technical support available, and collaboration amongst providers, families, individuals receiving services and the county board.

Please check each box to show that you have met each shared expectation, then attach documentation showing how the expectation has been met.

Regulatory compliance, customer satisfaction survey, Employee Retention and Recruitment participation, and Incidents adversely affecting health and safety expectations will be verified through documentation by the Stark DD provider compliance unit.

Once confirmed these expectations have been met, a committee of current Gold Star Collaborative providers and county board staff will review the application with the applicant in order to approve or ask for additional information. This will give the applicant the opportunity to ask questions and receive peer guidance from providers that have already met the gold star shared expectations.

Regulatory Compliance expectation - Provider has not had a suspension of certification in the past 6 years

Customer Satisfaction Survey expectation - These surveys would be completed by people served or their representative that receives services from the provider. Providers achieving the Gold Star designation will also have a reference (family or individual) willing to speak to others about the agency’s or independent provider’s services.

Staff Training Model expectation – The provider will demonstrate that they are in support of improved training models and creating a work environment that supports DSPs and themselves as professionals by taking part in the creation of (or providing) training that meets and exceeds current rule. Additionally, any employed DSPs will have possession of or access to their training file as part of their professional certification in their career.

Positive Community Culture and Innovation expectation – The ability of the provider to demonstrate how they have created and maintained a positive culture and have made strides toward innovation in the industry

Incidents that Adversely Affect Health and Safety expectation - Provider must be in good standing with MUI/UI preventative measures completion, maintaining MUI/UI logs and analysis forms, and have a consistent policy for working through MUI/UI occurrences (please submit policy to confirm requirement)

Safe and Informative Individual Service Transfers expectation - Providers assure safe and informative transfers of individual services from one provider to another by informing someone’s new provider of all pertinent information. This includes submitting comprehensive documentation of a person’s medical needs, medical appointments, important historical information, health and safety needs, community membership information, and other information important to assure the individual is healthy, safe, and has a successful transition. It is suggested providers use the provider DODD transfer checklist (please submit policy/documentation to confirm requirement)
